This simple questionnaire will get you to think about the effects that your current lifestyle may have on your health and wellbeing, and what small steps you could take to improve. Please note that this is a general test of wellbeing and should not be treated as a substitute for professional medical advice.

Task
Answer the questions as honestly as possible. Once you have completed the diagnostic, add up your score using the scoring guide provided and read the relevant Interpretation section to see how healthy you are.
1. Do you smoke?

b. I used to smoke but have given up, or I only smoke occasionally.
c. Yes, I am a habitual smoker.
2. Do you eat five portions of fruit and vegetables daily?
a. I try to, but usually only manage around three portions.

Interpretation
21 to 27: Well done, you seem to be in good condition and are well aware of the importance of looking after health. Keep doing what you are doing, but don't be too hard on yourself if you indulge or lapse once in a while.
Between 15 and 20: Not bad but there is some room for improvement. Review those questions where your score was particularly low and consider what changes you could make to improve.
Less than 14: Your unhealthy lifestyle could lead to some major health problems if steps are not taken to improve your wellbeing. However, small and simple changes will soon make the difference.

If you have identified any areas for improvement, put together an action plan and timescale for reaching these goals. Make sure they are achievable, otherwise you may get disheartened and abandon them completely, e.g. cutting down on the number of cigarettes smoked per day from ten to five may be more achievable than giving up entirely.
