Intentionality - Inventory Assessment

You may use this form to identify the intentions that are currently most important for you.


Place a checkmark in the column that signifies the level of importance of each item for you at this moment in your life, with 1 being “relatively unimportant at this time” and 5 being “very important.” Add your own or additional intentions in the spaces provided.


Try not to have more than three “5’s” at any one time. The key to intentionality is focus, and focusing on too many priority intentions dilutes our effectiveness at being intentional. For each “5”, write a paragraph (or several) that describes your intention in explicit detail.


Remember, this is simply a snapshot in time, a reflection of where you are now in your life. You may wish to do this exercise annually (or even quarterly) as priorities and circumstances in your life change.


My intentions at this moment are to be:

  1. Physically fit and healthy?

  2. Financially independent ?(or successful, or solvent)

  3. A great and loving (spouse, mother, father, sister, brother, daughter, son, friend, partner – you name it)

  4. An effective communicator?

  5. A better listener?

  6. Well-respected in my (community, profession, work environment)?

  7. Well-educated (well-read, degreed, a “Renaissance Man or Woman”)?

  8. A world traveler (or adventurer)?

  9. A successful writer (author, poet, playwright)?

  10. A successful artist (painter, sculptor, photographer, other)?

  11. A successful musician (or composer)?

  12. A visionary leader?

  13. An effective manager (executive, supervisor, coach, mentor, teacher, spiritual leader, HR director – you name it)?

  14. A better team player (team leader)?

  15. A successful business person?

  16. A masterful coach?

  17. A more grounded, centered?

  18. Living with greater ease and simplicity?
